OUTDOOR KENNELING RELEASE

I, ________________________________________, owner of ______________________________ do hereby authorize Garrett PetMinders to kennel said pet(s) in an outdoor kennel when I/we are away under the conditions set forth below:

In authorizing this mode of pet care, I/we take full responsibility for any and all occurrences of an unfortunate nature that may happen while my pet is kenneled outside, including but not limited to escape of the pet, theft of the pet, injury to and/or death of the pet while it is kenneled outdoors. I/we also completely and comprehensively absolve Garrett PetMinders, its owners or employees or any duly authorized agent, from any and all liability or injury which may result to humans, animals or material goods and property from kenneling my pet outdoors. Further, I/we specifically agree to waive any and all claims against Garrett PetMinders, Joseph R. Clark, Jr. and/or Kathleen Gibbs, their relatives, heirs or assigns now and forevermore for occurrences or injuries to my pets or property as related to kenneling it/them outdoors and for any other as yet unforeseen circumstances for the duration of this or any other pet sitting event.
This form will be retained on file by Garrett PetMinders and will be used to guide our pet sitting services with the pets named above. However, Garrett PetMinders reserves the right to house, kennel and/or care for pets as and how they deem best and acting “in loco parentis” for the above named owners of the pet(s).
UNDER NO CIRCUMSTANCES WILL GARRETT PETMINDERS ALLOW YOUR PET TO STAY OUT OVERNIGHT OR IN RAIN SLEET, SNOW OR OTHER ADVERSE WEATHER CONDITIONS, NOR WILL GARRETT PETMINDERS CHAIN, ROPE OR STAKE OUT A PET UNDER ANY CONDITIONS OR FOR ANY REASON, REGARDLESS OF AN OWNER’S DESIRE OR HABIT.

I, ________________________________________, have read and understand the terms of the Outdoor Kenneling Release and take full responsibility for any consequences of this action and decision on my part. Understanding this, I authorize Garrett PetMinders to kennel my pet in an outdoor kennel as described either above, in notes to Garrett PetMinders or as described in Garrett PetMinders Client Intake. Further, I agree that Garrett PetMinders may use its best judgment regarding outdoor kenneling of my pet as necessary.
